We are providing you with sprinkler test cards to enable you to record the results of weekly tests and checks which should be carried out. This may be a condition of your insurance - please refer to your policy documents for full details.

Please complete the test cards below and return them to sprinklerreview@aviva.com as soon as practicable after 31st December/year end on an annual basis.
