Information that could help you complete the death claim form:
- Reference number (if you have it) – you can find this in letters or documents from us. Or it may be the reference number on your bank statement Direct Debit.
- The death certificate – either a full death certificate or coroner’s interim certificate
- Details of deceased’s estate – for example, details about their Will or any surviving family
