Removing a member from your company policy

Before you go on

Please note, you can only complete this form if you are the group administrator. This will be the person who has been nominated to administer the policy on behalf of the company.

If you’re not the group administrator, please refer to your company contact.

If you remove an employee, any dependents will also automatically be removed from the policy.

If you want to remove more than one employee, please fill in a separate form for each person.
